There is nothing we can do about medical society guidelines. Just because you consider a procedure surgical or not doesn't mean they do. What do you want me to do, phone up the American College of Rheumatology and tell them they're wrong?

Please do not shut off your brain when we get review comments back. As you should know by now, many of our reviewers are idiots. If you change the script to match the reviewer's wacky ideas and not the reference, we're only going to have to change it back later.
I should probably deliver this rant in person...
