Well, it's "into the breach" in the St Crispin's Day speech in Henry V. Yeah, what did that Shakespeare guy know? We can run a Google plebescite:
"into the breech" -- 295,000 results
"into the breach" -- 9,270,000 results
I have been running parallel Google searches to check on the relative frequency of two possible terms for years, and never thought of naming the procedure, or even considered that it needed a name. But of course it does, and of course it has one. Perfection.
